Connecting communities to care
The U.S. health system is at a breaking point. Staffing shortages, fragmented care, and widening inequities are leaving millions without consistent, trusted support. Plus, up to 80% of health outcomes are shaped by social determinants of health outside the clinic — housing, transportation, food access, education, cultural navigation, and trust building with communities. Solutions to overcome these gaps in care are underfunded and under-integrated into health strategies. This offers us an opportunity to reimagine how health is supported and led in communities nationwide.
The Health in Action Collaborative brings together committed cross-sector leaders to identify, amplify, scale, and support some of the most promising and innovative solutions that better connect communities to quality care — and mobilize collective action around them.
