The Ingenious Way That Those with Traumatic Brain Injuries Are Getting Back on Their Feet

The news seemed like a death sentence: In 1998, Anne Dunlap, a pretty young woman in Delaware, suffered a traumatic brain injury in a car crash, severely impairing her daily functioning. Even with physical therapy, she struggled to juggle multiple basic tasks at one time. Walking while talking, for instance, had to be slowly relearned during 16 years of lessons at a clinic.
At least, that is, until Cole Galloway, a professor in the physical therapy department at the University of Delaware in Newark and the inventor of the Go Baby Go cars for disabled children, talked with Dunlap about what she wanted. “I was in a room with Anne and a bunch of other therapists,” Galloway tells NationSwell on a recent afternoon as he strolled the campus. “Someone asked her, ‘Hey, Anne, what would you like to do?’ She said immediately — she’s a very fashion-oriented kid — ‘Oh, I want to be in sales. I want to work at a mall.’”
Everybody in the room had heard this answer before and moved on, as it was just another routine blank to fill in. But Dunlap’s wish caused Galloway to wonder: Could physical therapy based in real-world actions — not exercises in an antiseptic rehab clinic — improve recovery time? To test his theory, he invented a harness system that could be rigged up on the ceiling of a small kiosk, giving Dunlap enough support to be fully mobile. Within a matter of weeks, the Go Baby Go Cafe opened. Dunlap’s new physical therapy regiment involved scooping ice cream, making coffee and taking cash at the pop-up store’s register.

“We’ve spent hundreds of millions on rehabilitative robotics, but very few, if any, have ever jumped out of the lab and into the community. That’s how difficult it is to design rehab technology for the real world from within the traditional lab,” he declares. “We have to look at things that have never been considered: aesthetics or performance in the real world. We should be going to Starbucks and asking, ‘What do you need to work here?’ then build a design to allow someone to serve a latte while standing, work on their arm or hand function. That’s the new engineering feat: to build laboratory-grade change in the real world.”
At the Go Baby Go coffee stand, the aroma of freshly ground espresso beans hovers in the air as employees hand out single and double scoops of vanilla, butter pecan and black raspberry ice cream. The only thing setting it apart from java bars at other college campuses? Its harness system, which looks like a four-poster canopy and is built with aluminum and steel bars. Following Galloway’s past success with low-tech materials (purchased from Toys ’R ’Us), he aimed for a model that could be cheaply reproduced.
“We’re working really hard to build stuff that’s simple,” Galloway says. “We prototype lightly so we can get it into the community and make it commercially available.”
An added feature of this system, like Go Baby Go’s cars, is its full-range mobility. Rather than limiting a person’s movement to one straight line, Dunlap is free to go in any direction within the enclosed 50-square-foot space, held up by the bars overhead. And the whole setup is portable, meaning it can be taken down from the science building’s lobby and reassembled anywhere.
“It’s very easy to move around in. It doesn’t feel like there’s any kind of drag, and the harness feels light when I’m moving,” Dunlap tells the Newark (Del.) Post. “I feel comfortable and liberated because I’m secure and protected, and I don’t have to worry about falling.”

Galloway, who got his start in physical therapy because he “loves movement” of all kinds — from the simple stuff like walking and sitting to ballet and gymnastics, hopes the harness system will have applications for people from all walks of life. Beyond physical therapy, the support system can assist anyone that needs extra support or balance, particularly children with developmental disabilities and the elderly. Serving as an interim device between relying on a walker and being confined to a wheelchair, the harnesses could be set up in school playgrounds, food trucks, family living rooms and workplaces.
Studies of Go Baby Go’s effectiveness are currently being funded by the National Institute of Health, the National Science Foundation and the U.S. Department of Education. Yet Galloway describes himself as a “pretty reluctant researcher.” While his lab prefers to see his prototypes out in public, they know a device works when they hear both positive feedback from the people who need it and receive solid data from his lab.
“We see no reason to wait for years of research to tell you that [with this device] a person who can’t stand or walk on their own has the potential to actually work in a cafe,” Galloway says, “Especially when your grandma would say today, ‘That’s gonna work.'”
Dunlap will tell you the same thing. She’s confident in her progress, so much so that she recently went down to her favorite coffee shop — not for a latte, but to drop off a job application.

This Common Sense Program Could Be the Future of Mental Health Care Nationwide

What would you do if your teenage son or daughter began expressing paranoid thoughts? Jumbling their sentences? Exhibiting bizarre behavior? Few parents know the warning signs of psychosis, but one joint effort in Connecticut is aiming to change that.
The state’s Department of Mental Health & Addiction Services and Yale University are pioneering MindMap, an outreach program designed to catch the early signs of a young person experiencing a psychotic episode, lessening the chance of self-harm or endangerment to others.
Mindmap’s creation is the result of university psychiatrists discovering that patients who received early treatment at community clinics reduced their risks of future psychotic breaks. Their findings also show that those who get help early on remain employed or in school 92 percent of time, compared to just 67 percent when receiving standard treatment. Even better? Three out of four avoided hospitalizations in the first year (as opposed to half), saving taxpayer dollars.

Patients receiving treatment at Yale’s Specialized Treatment Early in Psychosis (STEP) clinic are set up with a team of caseworkers, which coordinates medication, counseling and social skills training for the patient and education for their family, whose guilt or frustration at the disease can often exacerbate symptoms. (The lack of vital long-term care has been identified as the reason why our mental health system has largely failed after state-run institutions shuttered in the 1960s.)
“Traditionally, people show up to psychiatric care after many years of suffering and poor functioning. Maybe they got psychiatrically committed in a hospital against their will, maybe they were picked up by the police or became homeless,” says STEP clinical director Jessica Pollard. “We try to catch people as soon as they have noticeable symptoms, whether it is full-blown and diagnosable psychosis or signs of something to come.”
Hallucinations, delusions, disorganized speech or excessive motor activity can be caused by, say, stress or substance abuse, but they can also be symptoms of schizophrenia, bipolar disorder or severe depression. And while diagnoses have improved, mental health centers still struggle with the stigmas associated with psychosis.
“People don’t tend to show up on their own for care,” Pollard says. “They’re really embarrassed. They don’t want anyone to know.”
As many as one in every 29 people will experience some form of psychosis — generally defined as losing contact with reality — within their lifetime, according to one study. Psychiatrists know that most people have their first psychotic episode in their late teens or early twenties and reaching people at this early age is key, Yale’s psychiatrists say. It’s when the risk of relapse into psychotic episodes is highest and when two-thirds of suicides triggered by the disease occur.
“The model is a pragmatic, effective and economically feasible,” Vinod Srihari, professor of psychiatry at Yale School of Medicine, says in a statement. “The message is simple,” he adds. “The earlier, the better.”

How a Few Legendary Rappers and 1 Cool Doctor are Creating Better Health Outcomes for Inner City Kids

Dr. Olajide Williams is a neurologist and a hip hop fan. Doug E. Fresh is an critically-acclaimed rapper and an avid health nut. When the two paired up to produce, “Stroke Ain’t No Joke,” a song aimed at educating young people about the warning signs of a stroke, in 2005, it was a match made in heaven. “I would be working all day here, then head to Doug’s studio and work all night until we came up with the song,” says Williams. “It’s still my favorite song to this day.”
The song’s release led to the founding of Hip Hop Public Health, a campaign that uses animation, songs and live performances to educate low-income children and families about healthy living. Since 2008, the group has performed in more than 150 schools, teaching an estimated 44,000 kids about strokes, obesity, diabetes and Alzheimer’s disease.
Hip Hop Public Health recently launched an ambassador program, placing their materials online and encouraging educators and artists from around the world to use its model.

Forget Calculus and Gym Class. At This High School, Students Are Trained for Workplace Success

What is the purpose of high school?
At Camden County High School (CCHS) in Georgia, the school employs a “career technical” approach: using various academies aimed at different career fields to create a pre-professional, engaging environment for its almost 2,800 students.
All freshman are enrolled in the Freshman Academy, where they’re introduced to the curriculum and get acclimated to the school, as well as take most of the traditional academic core. From there, students pick one of the five career academies to enroll in, where they receive first-hand experience from people in that respective field, reports the Atlantic.
In the Government and Public Services Academy, students can follow the law and justice curriculum and take a class with Navy-Kings Bay NCIS official Rich Gamble. In Gamble’s class, students are trained in appropriate investigative procedures and court room preparation.
For those students looking for more technical work, there’s the Engineering and Industrial Technology Academy. This field includes a wide variety of careers, but CCHS covers many, including woodwork, welding, auto-repair, electrical work, computer-aided design and robotics program. Learning doesn’t just take place in the classroom; students actually sell products and perform services for the community, too.
Within the Health and Environmental Sciences Academy, students interested in the medical field can bring textbooks to life as they diagnose and care for dummies. The models are also used as test prep for certification exams.
There’s also a Fine Arts Academy (which covers all facets from theater to cooking) and one for Business and Marketing, where students learn the keys to success in the corporate world.
At CCHS, students are shown that there’s more to high school than just surviving class: a thriving career.

Meet the ‘Entreprenurses’ Behind a Clothing Line That Benefits Low-Income Families

Two nurses working in a neonatal intensive care unit have dubbed themselves “entreprenurses.”
To help the babies and their families at the Broward Health Medical Unit in Fort Lauderdale, Fla., Amanda Dubin and Kelly Meyer started a baby clothing company that helps needy families. Luc&Lou donates a onesie to a needy family for each one they sell and also supports nonprofits that benefit low-income families with newborns.
The design feature the tiny footprints of a 29-week-old infant that Dubin and Meyer cared for in the NICU. On one of the onesies, the footprints form the yellow rays of a sun and on another, a purple butterfly. “We were giving back to these little babies, and we wanted to really do it on a larger scale,” Meyer tells the Sun Sentinel.
Dubin says that they were inspired by the fighting spirit of the preemies they care for. “If they can do what they do, we can do anything.”
Now, Luc&Lou onesies go home with every “welcome to the world” package the Healthy Mothers, Healthy Babies Coalition of Broward County gives to low-income mothers of newborns. Sales from Luc&Lou products also benefit Fort Lauderdale’s Jack & Jill Children’s Center.
Meyer and Dubin have sold about 400 onesies so far and aim to expand. “We will always be nurses,” Dubin says. “That’s who we are. But we want to go bigger so we can help more people.”

With Books in Hand, These Students Are Going on a Ride

There’s a movement traveling through school classrooms across the country. Literally.
Stationary education is becoming a thing of the past as schools are discovering the benefits of blending exercise and learning. Through the Read and Ride Program, health- and grade-boosting exercise bikes are becoming fixtures in schools — and they’re proving their worth.
It all began five years ago at Ward Elementary School in Winston-Salem, N.C. Instead of desks, Ward has an entire classroom filled with exercise bikes. Periodically throughout the day, teachers will bring their students to the room to ride and read.
Not only does the program encourage and promote reading at a young age, the exercise factor improves students’ brain functioning, too. In 2010, Ward Elementary students who were in the program achieved an average 83 percent reading comprehension, while those who weren’t averaged just 41 percent, reports Fast Company.
Due to these standout results, solo exercise bikes are being added to classrooms to use as a “reward” for students or to just allow them to release some excess energy.
“Riding exercise bikes makes reading fun for many kids who get frustrated when they read,” program founder Scott Ertl tells Fast Company. “They have a way to release that frustration they feel while they ride.”
Looking beyond the educational benefits, the bikes also provide great exercise for kids who are confined in a classroom for six hours a day.
“Many students who are overweight struggle with sports and activities since they don’t want to always be last or lose,” Ertl explains to Fast Company. “On exercise bikes, students are able to pace themselves and exert themselves at their own level — without anyone noticing when they slow down or take a break.”
Since 2009, the Read and Ride program has gone national with chapters in 30 other schools. Russell Jones Elementary in Rogers, Ark., is one of those schools. In 2011, students who were a part of the program had an average growth rate of 113 points, whereas those who weren’t scored an average of 79 points.
They say that reading can take you anywhere, so the only question is: Where do you want to go?

The Health Care Company That Lets Its Shift Employees Choose When They Work

Consistently ranked as one of the best companies to work for, WellStar Health Systems is strengthening its reputation with its new online scheduling tool: Smart Square.
Named to Working Mother’s 10 family-friendly employers list, Fortune Magazine’s list of top 100 employers and a six-time recipient of the “When Work Works” honor from the Society for Human Resource Management, WellStar is well-recognized as a flexible and accommodating employer, according to National Journal.
The nonprofit is a health care network employing more 12,000 in Marietta, Ga. With 83 percent of it employees being female, WellStar offers services to ease the balance between work and home life. In addition to a traditional pension plan, WellStar provides an on-site day care center and a concierge that coordinates dry cleaning pickups, oil changes and grocery shopping.
Smart Square, though, is what’s really transforming the lives of WellStar workers. The company’s nurses work long shifts and advanced knowledge of work hours is one of the most useful pieces of information employers can provide as it gives the employee time to schedule home duties. This is exactly what Smart Square helps with.
Erica Kilpatrick is a WellStar nurse who also juggles home life and caring for her mother, who is currently in the hospital. Due to this hectic lifestyle, Smart Square gives Kilpatrick the chance to plan her three 12-hour shifts according to her needs.
“Smart Square equalizes everything,” Kilpatrick tells National Journal. “I get called to where the greatest need is.”
How does it work? Up to eight weeks in advance, employees can start signing up for working shifts through Smart Square. (While they have the flexibility to work hours of their choosing, all employees must work the required holiday and weekend shifts.) Managers have the final say on scheduling approval, but the process is collaborative between employer and employee — rather than a dictation of hours that must be worked.
One disadvantage of the system? It can’t account for the unpredictability of life in the medical profession. Since the appropriate nurse-to-patient ratio must be followed, during hectic times, employees may be called in to work with only one or two days’ notice.
Despite this, though, Smart Square is still very efficient, especially since the app allows the nurses to manage their shifts and sign up for additional ones.
“It definitely has opened up that ability for the nurse to have a lot more control over his or her schedule,” Kris Betts, WellStar’s assistant vice president for workforce engineering, says. “It’s not, ‘I’m hiring you specifically for Monday, Wednesday and Friday, and you’d better be there on those days.’ This way is a lot more fluid.”

Ancient Native American Ceremonies Help Soldiers Overcome PTSD

You’re probably not aware, but about 1 percent of veterans are of American Indian or Native Alaskan descent.
While this group is just a tiny percentage of our Armed Forces, Native American veterans are two to three times as likely to experience PTSD as white veterans, says Dr. Spero Manson, Ph.D., who leads the Centers for American Indian and Native Alaskan Health at the University of Colorado’s School of Public Health.
Why does this group suffer mental anguish more than others? Manson, who is a member of the Pembina-Chippewa tribe, thinks it’s because Native Americans are more likely to spend more time in combat than soldiers of other ethnicities. “The greatest predictor of trauma among veterans is, in fact, exposure to combat,” he tells Colorado Matters.
Although the issues faced by veterans of Iraq and Afghanistan are unique, Manson believes this problem isn’t new, extending back for as long as there have been warriors. “The returning warriors of that time came back to their local villages and communities exhibiting many of the same symptoms that veterans today, who have seen combat, do,” he says. “They’re irritable, quick to fight, they distance themselves from others. They’re very difficult to reintegrate into their communities.”
Manson believes the ancient ceremonies tribes developed to address these problems can be helpful to today’s soldiers. He cites the Lakota Wiping of Tears, “where tears are symbolically brushed from the cheeks,” as being helpful.
Manson’s own son returned troubled after serving in the Marine Corps and finally got back on his feet through a mixture of tribal and traditional medical interventions. “We just have to figure out how to…support them in the process,” Manson says.

We’ve all heard of the benefits of living in walkable cities — not only are they good for your heart and body, but foot-friendly communities help reduce environmental impact as well. And now, new preliminary research finds that walking improves brain function, too.
In a small-scale study, Amber Watts, a professor of psychology at Kansas University in Lawrence, discovered that taking a stroll — whether to the bank or to simply enjoy a beautiful day — helps keep your mind active and fresh. Results showed that test subjects (who were older and included some with Alzheimer’s) who resided in more pedestrian-friendly ares actually scored higher on cognitive tests, according to Fast Co. Exist.
Watts used the mapping software Space Syntax to gauge the areas with the most connectivity, which measures the amount of places within a half mile of a participant’s home, and integration (how difficult and intricate navigation of the area is).
“Higher connectivity across the board is associated with better cognitive function. It could be that there’s more places to walk to, more people to socialize with, more opportunities to get there,” Watts tells Fast Co. Exist.
Because of the findings, Watts is preparing to conduct a large research project on how walkability affects cognition. For it, she plans to strap pedometers to 100 older adults, including those with early dementia.
“I wouldn’t say that moving to a walkable neighborhood will prevent you from getting Alzheimer’s,” Watts explains. “This could be a relatively small contribution, but it could be important. We can’t change our age, we can’t change whether we have genetic alleles that put us at risk, but we can change how we live.”
